Fill out the form below if you or someone you know has encountered fraud, waste, or abuse related to PerformRx, members, pharmacies, prescribers, vendors and/or employees.
Please complete the form with as much information as possible.
You do not have to provide your contact information. An investigator from our Corporate and Financial Investigations department reviews each referral, and may contact you for additional information, unless you have asked us not to contact you.
